On December 18th, 2024, at about 11:30 pm the employee parked loader in fines shed adjacent to hopper 2. Employee was disembarking loader to clear metal detector fault. On reaching the ground the employee felt a pain above EE's right knee.

On 10-10-24 at 1:20 PM Employee struck their left-hand ring finger with a pry bar.

On October 4, 10 AM employee received a laceration on EE's right wrist above EE's glove cuff.

Employee was packaging material. Employee turned around to leave area and tripped on pallet lift table, fell and hit head. Diagnosed as possible mild concussion. No overnight observation.

Employee was on top of bag house in mill 11/12 building replacing a motor. After 20 minutes on top of bag house employee felt dizzy and fell over from heat illness. Employee was responsive and alert. Was taken to hospital for evaluation.

Employee was operating haul truck. Truck was unloaded and driving down into the mine when EE felt pain in lower back. Employee stated was unsure if it was due to twisting while reaching for something or from a bump in the road.

Employee was climbing off of ladder. Employee missed a rung and fell approximately 3 feet to the ground landing on their rear end. Employee sprained knee sometime during the fall.

Supersack of material tipped over and contacted miner's ankle causing bruising.

Employee was loading tanker truck. It was rainy and wet. Employee slipped and fell on top of the trailer. Employee tried to catch themselves with their hand and strained wrist. Employee did not fall off of truck.

Employee was bent over at face to clean out drill cuttings from drill hole using a clan out rod. Tool weight is approximately 5 lbs. Employee felt a pull in their lower back. Improper ergonomics.

Employee was using a shovel to remove material under conveyor. Strained back while shoveling.

Employee tripped on uneven surface in mine and strained knee.

Employee placing pallet into palletizer. Pallet nail became caught on chain conveyor causing the pallet to stop suddenly and strain the employee's back.

Employee was stacking a pallet with 50lb bags higher then allowed which caused employee to lift bag overhead. Employee felt a pulling in belly but did not feel pain. Resulted in hernia above the belly button. Not reported to HR or EHS for months. Was not deemed work related until 8/31/22.

Employee was carrying cardboard that obstructed view of the ground. Employee tripped over and fell onto pallet press. Resulting in laceration to leg with 2 stitches and sprained knee.

Employee was walking and attempting to pass under low overhead. Employee did not get low enough and caught top of hardhat on beam causing neck strain.

Employee bent over to pick up full 2-gallon DEF bottle when they heard a pop in their back. Resulted in doctor visit with muscle relaxers.

Employee reached onto conveyor to remove fallen empty bag. The employee's left hand became entangled into the chain conveyor and passive transfer roller resulting in laceration to palm of left hand.

Employee reached overhead to open stairs of haul truck for pre-operational inspection. While pulling on latches, employee felt a pain in shoulder.

Employee was walking down stairs in plant. Employee missed the last step and stepped onto landing at base of stairs. Employee felt pain in left knee. Both the stairs and landing were free from trip hazards and debris. Employee stated: just was not paying attention.

Employee was manually installing roof bolts. Was in a man basket inserting mechanical roof bolts, using an impact to install bolts, and using torque wrench to tighten roof bolts per spec. Shoulder became sore in the process of setting approximately 80 roof bolts in a shift and was still painful 2 weeks later so employee reported incident on 8/16/2020.

Description of Events:Employee was working with another and performing housekeeping with a shovel at Tank 18. The employee went to reposition the small dumpster and strained back. Restricted duty.

Employee was loading a bulk truck. When trying to open a lid one of the hatches was bound up/stuck. The employee could not find a "hatch tool" and tried to manually force the latch open. Employee felt a pain in shoulder but finished the shift. Incident was reported the next morning when the pain had not gotten better.

Miner was attempting to remove a frayed piece of rubber from a haul truck tire by pulling on it. When they pulled they felt a "pop" in right shoulder followed by minor pain. Our mine Supervisor took the employee to a local med clinic and they receive a steroid shot and some minor restrictions for a sprain.

As an employee was dismounting a haul truck, the employee's foot slipped off the bottom rung causing them to make contact with the ground. The employees hard hat fell off during the fall causing a minor abrasion to the back of their head. The employee was given medical treatment to clean the wound, and a prescription muscle relaxer and pain medication.

Employee was preparing to ingress off the Trac-Drill when EE hit elbow on the grab handle.

Employee pinched finger while attempting to move a stretch wrapper.

Employee was digging out a plugged mill

Employee received burns on their arm while touching a journal inside a Raymond mill.

Employee was climbing in drill when foot slipped off ladder and pulled shoulder out of socket.

Employee slipped and fractured left ankle.

Hearing threshold shift in right ear.

Standard audiometric threshold shift in right ear.

Reportable hearing threshold shift in right ear.

Employee was using a pneumatic impact wrench to repair equipment. When air was supplied to the wrench, the front facing exhaust stream of the wrench stirred up debris which was blown into the employee's eye.

Employee felt a twinge in back upon dismounting a forklift.

Employee was filling 50lb bags and manually placing them on a pallet. As ee was removing a bag from the filling spout, the bag hung on the spout as ee was turning to place it on the pallet. This caused a groin strain.

Shipping operator reported knee gave out while climbing down Silo 23 gantry stairs after moving a truck from silo farm.

Maintenance tech moving barrels off of a pallet had foot catch between two slats of a pallet. Upon trying to free foot, the employee felt a pop in knee and it began to swell painfully.

Maintenance employee removing screen deck door felt pop in shoulder as they were forcing the door open. Door had been siliconed shut to prevent leakage.

Utility operator reports running over pothole's and ditches in the yard after heavy rains and woke up this morning experiencing significant back pain. Physician prescribed muscle relaxer and assigned time off work.

Employee reported severe shoulder/neck pain the day after hammering out the mill 03 feed system from bridged and plugged wet feed.

Shipping operator loading out of silo 22 had EE's left pinky pinched between the silo spout and the platform steel when EE tried to manually adjust the position of the spout for a poorly-aligned truck. After getting help from a maintenance employee, the spout was either accidentally raised or shifted into the beam, pinching the operator's finger.

Employee complained of pain in knee after working weekend climbing ladders, loading trucks, and getting on and off of equipment.

Employee reported wrist pain on day following date of injury. Employee was bagging treated product on date of injury. Employee sent for medical evaluation and prescribed work restriction and painkiller.

Employee was dumping a half-full carboy of wastewater (3 gallons) into a sink when they felt a sharp pain in their left shoulder. Upon medical evaluation, employee was diagnosed with a sprained shoulder and lower back injury.

Maintenance mechanic lying on side while welding had small piece of hot metal fall into left ear. Some temporary hearing loss has been apparent, full damage unknown at this time.

Employee walked to retrieve a gladhand lock out of the lock bin on the shipping dock. Upon turning around to go load out a truck, the employee felt a pop in his right knee. Pain and swelling ensued soon after. Employee was sent for medical evaluation and assigned a 7-day work restriction.

Miner stepping down from step stool missed a step, causing him to fall backwards and strike back of hard hat on floor. Miner was sent to emergency room for evaluation, diagnosed with contusion, minor head injury, and minor neck/back sprain. Was prescribed painkillers and given work restriction of three days.

Temporary employee handling pallets received splinter in right middle finger on Tuesday 03/18. First-aid conducted to remove splinter. On Friday 03/21 employee complained that finger was swollen and painful, employee was sent in for medical evaluation. A piece of the splinter was found remaining in finger, employee was prescribed antibiotic and work restriction.

Employee was helping to move a steel plate when the plate fell on his left foot, landing just behind the steel toe. He suffered a cut and bruise on the top of his foot.

Employee was shoveling material from screen deck. After shoveling felt pain in left hip. Went to doctor about it on 04/09/2013. was diagnosed with acute lumbar strain.

Employee was cleaning up a large amount of spilled dust, sweeping and shoveling approximately 4000 lbs. Noticed a pain in her left hip. Not better on Tuesday, April 2 (her next reporting day). Went to Doctor, diagnosed as lumbar strain.

Employee was bending over to pick up something that fell off his desk and experienced pain in back. Incident not reported until 1-24-2013.

Employee was reaching for stretch wrap material when he slipped and fell against the machine guard, resulting in bruising on right rib area. Employee was evaluated at doctor with x-ray and found no broken ribs. Returned to work with restricted duty.

Miners setting pump in left ramp noticed roof fall had occurred sometime over Thanksgiving holiday in adjacent ramp. MSHA was immediately notified.

Employee tripped and fell at dock area. Complained of back pain. Initially thought injury was minor but three days later (7/8/2011) had to go to hospital for back pain. Back pain treated with anti-inflammatory, muscle relaxant, and pain med and employee released. Employee reported back next scheduled shift (7/12/2011) with no restrictions.

Shipping operator was closing latch on bulk pneumatic truck hatch and caught end of left middle finger under latch, resulting in non-displaced fracture of finger tip bone.

Employee stated that he had become overheated the previous shift (6/23). Despite resting and drinking 4-32 oz bottles of Powerade, he said he was still having symptoms of heat exhaustion and insisted on being seen by a physician. He was transported to the local hospital ER and given fluids and medicine for nausea and headache. He was released at 1 am.

Received results of retest of annual hearing test indicating employee has reportable hearing loss in right ear. Only one ear & developed quickly so further testing is being done to determine possible medical issues or work-relatedness

Scaler exited scaling machine to check face. While doing so, passed by 206 lb rock leaning against face which tipped over onto his leg pinning him to the ground for 45 minutes. Radio was in cab of machine. Driller found him and freed him. Scaler was taken to hospital for assessment of leg. X-rays were taken no bones were broken. Diagnosis was contusion of right lower leg.

EE had parked at fuel tank.While descending from the truck,EE's right foot caught on the last step, causing him to hit the ground hard with his left leg.EE initially thought injury was minor and self-treated with ice and Excedrin.On 11/21/2008, swelling was still present so employee was taken to Doctor and was prescribed an anti-inflammatory medicine.

Loader operator was mucking out a heading that was shot the previous shift. He scooped up a bucket of material, raised his bucket approx. 3 feet and started backing up when (apparently) an unexploded stick of dynamite detonated. All miners were evacuated from the mine. There were no injuries or property damage. MSHA was immediately notified.

Front end loader operator was removing rock from the face. He noticed a large rock in the floor of the room. The operator checked the rock for any evidence of detonation cord and upon observing none, proceeded to use the bucket of the front end loader to ?flip? the rock over. When the operator ?flipped? the rock, he noticed an explosion.

EE was squating down to open the hatches on a bulk truck when he felt a burning sensation in his left knee. EE is scheduked for a MRI on 7/6/05 and the doctor will determine the extent of the knee injury.

EE was opening hatches on bulk trailer when he felt burning in right knee. EE was determined to have a torn meniscus on 12-21-04. Surgery performed on 1-5-05.

Employee was working underground operating the scaler when he got out of the scaler to grease the boom rollers on the equipment. Employee lost his footing and fell approximately 4 feet to the ground which was muddy and rocky surface.

EE WAS CHANGING TIRE ON BACKHOE. HE ROLLED IN THE SPARE AND STARTED TO REMOVE BAD TIRE. TIRE FELL ON LEFT HAND. HIS MIDDLE FINGER WAS SPLIT AND REQUIRED 9 STITCHES.

EE BELIEVED HE HAD LOCATED HYDRAULIC OIL LEAK. EE PLACED BARE HAND ON UNSUSPECTED LINE, PULLING IT AWAY FROM SUSPECTED LINE. EE ASKED OPERATOR TO PRESSURIZE SYSTEM IN ORDER TO POSITIVELY IDEN TIFY LEAK. UNFORTUNATELY, EE'S INDEX FINGER WAS PLACED PRECISELY ABOVE A PINHOLE LEAK. AT OVER 2,000 PSI, THE PINHOLE LEAK PIERCED EE'S FINGER & INSTANTLY FILLED IT W/HYDRAULIC FLUID.

EE REMOVED GUARD FROM HAMMER MILL, SETTING IT ON THE PLATFORM. LITTLE FINGER WAS PINCHED BETWEEN GUARD AND BOLT EXTENDING FROM THE MILL.

DUST BLEW IN EYE WHILE TAKING HOSE OFF.

ROCK FELL WHILE EMPLOYEE WAS AT FACE OF EAST PORTAL. ROCK STRUCK LEFT FOREARM.
