Employee was exiting the loader, and EE's foot missed the last step on the ladder. Employee fell to mine floor, landing on EE's back. Initial medical evaluation was for a sprain/strain, but a follow up appointment on 12/16/2025 with X-rays determined EE sustained a broken rib.

Were notified of a missing employee on 12/25/2023, employee discovered in the area around explosive magazines. Once discovered MSHA Hotline was called at 13:40 to notify of a death at the mine.

THE IE WAS REPLACING A LIFT JACK ON A TRAILER WHEN THE JACK SLIPPED CAUSING THE IE TO JERK HAND AWAY. THE "JERKING" MOTION CAUSED THE EMPLOYEE TO STRAIN THEIR R SHOULDER. INITIALLY IT WAS A REPORT ONLY, IE SOUGHT MEDICAL ATTENTION ON 8/14/2023 WHEN EE RECEIVED AN INJECTION MAKING THIS REPORTABLE.

On 5/26/2022 a OTR Driver walked into the entrance of the portal. Company truck #470 was entering the mine and bumped into the OTR Driver. The OTR driver was taken to hospital where they were treated and released. The OTR driver suffered a laceration to the back of their head.

The IE was heading back into the mine and failed to lower the bed on a Cat Articulated Haul Truck. The bed of the truck struck an I Beam support stopping the truck, causing the cab to roll on its side. The IE was wearing a seatbelt.

IE was working on the crusher jaw at Mill 31. The IE was in the process of tightening bolts while working from the 3rd step of a portable ladder when the ladder slid out from under EE causing EE to fall to the concrete below. The injured suffered 3 fractured ribs.

Employee was assisting Mill Operator in greasing the jack-shaft for the hammer-mill. After greasing the shaft EE leaned over to pick up the grease gun and yelled out in pain. Employee stated EE "threw back out" bending over to pick up the grease gun. Employee has a history of back problems.

Employee was getting into haul-truck 122, a CAT 769 rigid frame truck. EE opened the door, then turned around and was backing into the cab. When entering the cab EE's left foot either slipped or missed the walkway causing EE to fall to the left striking ribs on the left side on the handrail of a second truck parked next to it.

Employee reported on August 10, 2015 to his Supervisor that he hurt his back while driving the endloader the last week of July, 2015. Employee believes he twisted his back while pushing up product being unloaded from the barges onto the stockpile. It started out as an achy joint, but has gotten worse. Employee did not communicate this to anyone before August 10, 2015.

Taken to Clinic-Xrays & medications. Lift restrictions. Steering wheel flat&no tilt. Tried to simulate. Was suspect & 8/15 WC determined head & neck would be covered not shoulder.

Employee was dumping a hopper into a dumpster. Employee triggered the release with his hand while standing next to the hopper. The hopper tilted forward to dump and bounced back. When the hopper came back, the employee's finger was pinched between the hopper frame and tow motor. The employee's finger-tip was broken and lacerated. The finger required 7 stitches to close the wound.

A mechanic was changing the front tire on a 15 ton Ford dump truck. The mechanic removed the bad tire and leaned the tire against a hand rail. As the mechanic walked away from the tire, it fell and struck the mechanic in the back of the leg. This caused the mechanic to fall backwards, towards the falling tire. The incident resulted in a broken leg.

12/19/07 - Maintenance employee was walking up an inclined travelway when he stepped on a small stone (1") and felt a "Pop" in his knee. Employee did not believe it was anything and did not want a medical evaluation. Later, 1/3/08, the employee reported lingering discomfort in the knee; doctor evaluated and prescribed Celebrex and it became reportable at that time.

While cleaning and servicing a Great Western Screen, the employee put his hand down the discharge sock (17") and his finger impacted an airlock. The employee lost the fleshy front tip of his middle finger. The employee was taken to the hospital and treated for the wound. The employee failed to follow the documented company policy for Lock-Out/Tag-Out and was terminated.

Employee was installing the ductwork and was tightening down a nut when his ratchet slipped off of the nut. The employee lost his balance and fell backwards. The employee impacted a handrail support that tore through two layers of clothing and left two deep wounds in his upper right thigh.

Employee was pushing on a pallet that had stopped short of the end of the roll conveyor. Employee's stomach muscles were in a condition which allowed this last act of pushing a pallet to cause employee to be diagnosed with a hernia.

Employee was assigned the task of removing the liner from a furnace. Castable frefractory had formed around the bottom of liner and employee was using a hammer and chisel to loosen this and also a cum-a-long. Employee got up to get a drink of water when he had a sharp pain in his chest. He was sent to the medical clinic and was given muscle relaxers.

Heavy Equipment/Diesel Mechanic with over three years of experience was using a pry bar approximately 5 feet long to straighten a mirror bracket on an end loader. The bar slipped out of the bracket and struck the employee in the lip. Employee's lip was lacerated requiring stitches and lost time away from work.

Employee started shoveling at Belt 2 head pulley at the crusher. Approximately 45 minutes into the job felt shooting pain from left rib side to right side with shortness of breath. Employee stood there waiting for the pain to go away. Employee started shoveling again and felt the same pain again.

Two employees were in the process of setting the tension. The other was holding tension on spring with spanner wrench. EE was preparing to put a locking pin which holds spring in place, began to place the pin; wrench broke allowing the spring to release tension. Bolt cut through his glove causing a laceration on right hand in the fleshy area below the thumb. 8 sutures.

While employee was performing a maintenance activity at a height of approx 8 ft, the ee used a retractable tape measure as a pointing device. While pointing at a damaged roller, the tape became entangled resulting in laceration of right index finger, requiring six sutures.

AT SHIFT CHANGE, THE ON-COMING AND OFF-GOING OPERATORS WERE DISCUSSING SHIFT CHANGE INFORMATION WHILE STANDING OUTSIDE. DURING THIS TIME, THE EEFELT SOMETHING BLOW INTO HIS LEFT EYE. THE EE F LUSHED THE EYE TWICE DURING THE NIGHT WITH EYE WASH AND USED VISINE...REPORTED IT TO SAFETY COORD. ON HIS WAY HOME...AT 4:30 PM EE WENT TO LOCAL ER TO HAVE HIS EYE LOOKED, WAS HURTING.
