A 24" X 40' CHANNEL CONVEYOR SUPPORTED ON ONE END BY SCREEN FRAME 6' FROM GROUND. OPPOSITE END SUPPORTED APPROX. 6' FROM GROUND BY 175C MICHIGAN END LOADER AND LOG CHAIN. EMP DECIDED TO PASS F ROM ONE SIDE OF CONVEYOR INSTEAD OF AROUND LOADER. AS HE PASSED UNDER, THE LOG CHAIN SLIPPED ALLOWING CONVEYOR TO DROP, HITTING EMP ON BACK. CHAIN SECURED ON SIDES, ONE SIDE IMPROPERLY.
