General Shale Brick Inc · Struck against a moving object

Frozen tire tracks (6-8 inches deep) and normal winter ground conditions. Driver claims that bumps caused him to leave seat and strike rib on dump lever. Driver claimed to be wearing seat belt. No damage or defect found on truck.
