EQUIPMENT OPERATOR WAS USING AN IMPACT WRENCH TO REMOVE THE BOLT ON SEGMENT ON BUCKET CUTTING EDGE WHEN THE IMPACT WRENCH SLIPPED CUTTING THE RIGHT INDEX FINGER.
PLANT OPERATOR WAS USING A 4" ELECTRIC GRINDER TO CLEAN BREAKER APRON ON STEDMAN CRUSHER. WEATHER WAS COLD, OPERATOR SAID SAFETY GOGGLES WERE FOGGING UP, SO HE CHANGED TO SAFETY GLASSES, WHICH LET DIRT AND DUST GET INTO HIS LEFT EYE.
