Employee noticed a rock up in guard of jack shaft on surge bin. Employee noticed rock was barely on the cross member inside the guard. Employee hit back of guard to knock rock out. Rock went into coupler of jack shaft & sent rock into the guard. Guard caught employees hand & expanded metal went through employees glove & cut employees right hand.

Employee pinched his right ring finger between a return roller and the frame of a conveyor causing a laceration requiring 3 stitches.
Employee was cutting a piece of rubber with a utility knife and the knife slipped causing a laceration to his leg that required stitches.

Employee was bitten by a brown recluse spider on the top of the right hand. Employee originally did not report the incident and turned it in on his own insurance. When notified that it happened on company property, we turned it into worker's compensation and reported.

loaded truck on haul road ran into dozer causing truck to tip on driver side. Driver got out walk- ed arround for 15 min later complained of knee & abdomen pain went hospital. Treated for bruises. Was observed for 23 hrs. EE claims blacked out Dr put EE on restricted him from operating mach- inery and driving until tests are completed.

EMPLOYEE WAS GUIDING A CONVEYOR BEING LOADED ONTO A FLATBED TRAILER. AS THE LOADER HANDLING THE CONVEYOR TILTED ITS BUCKET, THE CHAIN RIGGING SLIPPED CAUSING THE CONVEYOR HEAD PULLEY TO JUMP, PINCHING THE EMPLOYEE'S HAND BETWEEN THE HEAD PULLEY AND THE TRAILER HEAD RACK.

EE WAS REMOVING STUCK FUEL CAP FROM TANK ON FUEL TRUCK, WHEN FROZEN CAP RELEASED, EE FELT A SNAP IN HIS LOWER BACK ON 12/4/97. ON 12/12/97 EE EXPERIENCED DISCOMFORT & NUMBNESS IN HIS LEFT LEG. ON 12/15/97 EE WENT TO DR & WAS DIAGNOSED WITH MUSCLE SPASMS THAT AFFECTED NERVES IN HIS LEFT LEG.

EE IS NOT SURE WHAT SPECIFIC JOB HE WAS DOING. EITHER RUNNING A LOADER OR LIFTING SAMPLE BAGS. ONE OR BOTH OF THESE JOBS COULD HAVE CAUSED HIM TO SUFFER A BACK STRAIN WHICH BECAME ACUTELY PAIN FUL THE FOLLOWING MORNING.
EMP WAS USING A CUTTING TORCH, CUTTING A PIECE OF IRON OFF AN I BEAM OVER HIS HEAD. A SPARK DROPPED INTO HIS RIGHT EAR BURNING A HOLE IN RIGHT EAR DRUM.

EMPLOYEE WAS POSITIONING A STEEL PLATE IN PREPARATION FOR WELDING. THE PLATE SHIFTED AND PINCHED HIS RING FINGER ON HIS LEFT HAND, CAUSING A PARTIAL AMPUTATION.
