Quality Stone Quarries, LLC · Caught in, under or between a moving and a stationary object

Employee had their left hand between the rock & chopper table. When second employee cut the rock, index and middle finger were smashed. Minor damage to middle finger index finger required stitches and a splint. Follow up with dr. in 2 weeks.6-12 week recovery time.

Loader hand found EE on the ground disoriented. EE asked for help and to go to the hospital. The ambulance was out on call so some of the other employees took EE to the ER. It was only after everything settled down that we found a rag and some window cleaner. Best we can tell EE got out to clean mirror or window and fell missing a step or handle.

Co-worker was on skid steer while EE was trying to take bucket off because it was too tight. EE instructed co-worker to raise the bucket up. It was 3.5 feet in air and EE walked in front of the bucket and pried the lever on the coupler. The bucket fell severing the big toe and the 2 beside it.

Arrived not feeling well, became ill and unresponsive and was taken to the hospital. He ultimately passed away from stroke. His illness was not work related.
