An unintentional roof fall was discovered between break 33 and 34 in the #2 entry the fall was 30' land X 19' wide X 4'6" high. This fall will not be cleaned up and is timbered or cribbed on all sides and dangered off. The fall does not effect travel or ventilation.

Rock fall was discovered in the intersection at 50 break in the #3 entry along the #1a belt. The fall was 5' high x 20' wide x 30' long. No injuries and no ventilation controls or equipment was effected. This fall will be not be cleaned up and the area has been dangered off.

An unintentional roof fall occurred on the #4 belt in the #4 entry 30 feet inby spad #1025between the blocks. estamated size of fall 15'L. X 18' W. X 4-5' H. Area had been supported with 4' fully grouted resin bolts. No injuries occurred, ventilation was not interrupted, conveyor belt was caught. Fall will be cleaned up.

An unintentional roof fall was discovered in the #2 entry (return) at #36 break spad #676 adjacent to the 1A belt. Size of fall was estimated to be 30'L. x 19'W. x 4-5'H. Fall area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught. Ventilation was not interrupted. Fall will not be cleaned up.
