The employee was bolting, went to lower the basket, smashed hand between the spinner and the railing on the basket.

The employee was walking down the steps of the scalp screen, slipped and fell, hitting hand on the end of the handrail.
The employee was doing a workplace exam, when a small piece of loose fell and struck the employee. EE was diagnosed with a fractured rib on 11.3.22.
Employee stated was climbing down from bin 2 and rolled ankle while stepping down.

Employee stated was moving a guard while making repairs and felt a pop in their hip. Employee received medical treatment and was given work restrictions on 09/27/2021.
Employee stated EE was hand scaling, when a rock broke loose EE felt pain and numbness in EE's arms. On 9.10.2021 the employee was given work restrictions.
Employee stated was stepping out of the anfo rig and missed the step and twisted ankle.
Employee states that while driving haul truck employee hit a bump in the road that caused the air seat to bottom out, jarring employee's back. Employee started physical therapy on 8/17/2021 which made it a reportable incident.
Employee reported on 7/14/2021 that EE's back was hurting and had been bothering EE since the week of 6/28/2021, when EE was operating various pieces of equipment. Employee stated EE's back hurt from climbing in and out of equipment. Employee received medical treatment on 7/14/2021 was released to full duty and again on 7/27/2021 when EE was given work restrictions.
Employee stated that while mounting a haul truck, employee bent over to pick up the coffee that employee had placed on the bumper, lost balance, and fell off the truck.
Employee stated that while operating a loader, the bucket struck a toe on the floor and caused elbow to be stuck between the arm rest and the machine controls, resulting in elbow pain. No medical treatment until 04/12/2021.
Employee was walking towards EE's vehicle because the mine shut down due to snowy weather conditions. EE slipped and fell on the snow in the parking lot near EE's vehicle. Physical therapy recommended to start on Feb 15.

Employee stated EE was shoveling on the catwalk of the VSI when EE felt a pain in EE's lower back. Employee received medical treatment on November 6.
On 1/20 during the workplace exam, it was discovered the Gazebo Lake located above the mine had emptied through a geological formation its entire contents into the mine. Date and time of the occurrence is unknown, no weekend work. No injuries from this accident.

Employee stated was using a pry bar to clear material from a chute when the bar slipped and employee caught a finger between the chute and the pry bar.

Employee stated ee was working on a hose on machine, felt a sharp pain in knee while walking around machine. No restrictions on initial medical examination, treatment on 2/21/2018 resulted in injury becoming reportable.
Employee stated stepped out of truck to check a face area, when EE turned around to get back in truck EE felt a pain in calf. No medical treatment was given until physical therapy started on Dec 15.
Employee reported feeling numbness in hands and wrist after hand scaling. Medical treatment on 12/14/2017 resulted in light duty.
The employee stated employee was picking up a belt roller and felt a pop in employee's back.

The employee experienced back pain over a period of time while operating loader underground. Employee reported the injury on June 7th and requested medical treatment, work restrictions were issued on June 8th 2016.
The employee pick up a tool turned around to take a step, felt knee pop and fell to the ground. (The employee is now receiving Physical Therapy.)

EE reported to have injury at work after working all weekend at his 2nd job. EE admitted to working at his shop 8-10 hrs barbequing on concrete at pumpkin patch. Minor lifting of pumpkins from bed of truck resulted in strain. Questionable as being work related.
EE alleges left arm & shoulder was hurting when he is operating the Mechanical Scaler.
EE alleges cervical strain from operating scaling machine.

EEs were loading a face round from a manlift, when a rock fell from the roof and struck injured on the shoulder and hip. The rock weighed approx 75 lbs, causing injuries to the shoulder and hip. EE was taken to the hospital by ambulance.
