Car drove through a stop sign, over railroad tracks, through a guardrail, through a fence and over a 4 foot berm into pit 8 of the quarry. 3 people in car were killed. (Not mine or mining related)

A EXCAVATOR WAS BROUGHT TO THE SITE WHERE A 60' FT 10" PIPE WAS TO BE DUG UP. IT WAS RAINING-FOREMAN TOLD OPERATOR TO SIT IN THE TRUCK. FOREMAN WAS LIFTING THE PIPE UP AND THE OPERATOR HAD RET URNED W/O THE KNOWLEDGE OF THE FOREMAN. ONE END OF THE PIPE FLIPPED UP APPARENTLY STRIKING THE OPERATOR KNOCKING HIM UNCONSCIOUS AND CUTS ON THEBACK OF HIS HEAD.

THE EMPLOYEE STARTED DOWN A RAMP WITH A EUCLID R-50 REAR DUMP. THE TRUCK STARTED TO FREEWHEEL AND WAS STEERED INTO A BUMPER PILE AT THE RAMPS END. AN AXLE BROKE WHICH PUT ADDED FORCE AGAINST T HE OPOSITE SIDE WHEEL LOCKS WHICH IN TURN GAVE WAY CAUSING THE TRUCK TO FREEWHEEL.
