EE WAS REMOVING OLD SECTION OF WALL FROM THE MILL. IN THE PROCESS OF HITTING THE TIN TO REMOVE THE NAILS A PIECE OF TIN WAS IMBEDDED IN HIS EYE & HAD TO BE REMOVED.

EE WAS GETTING READY TO WELD A PIN BACK IN PLACE ON THE CHAINS IN TH EMOUTH OF THE CRUSHE RAND WHEN HE STEPPED FROM ON EROTOR TO THE OTHER, ROTOR HE SLIPPED AND BENT HIS FOOT UPWARD BETWEEN TH E TWO ROTORS FRACTURING HIS RIGHT FOOT.

A EXCAVATOR WAS BROUGHT TO THE SITE WHERE A 60' FT 10" PIPE WAS TO BE DUG UP. IT WAS RAINING-FOREMAN TOLD OPERATOR TO SIT IN THE TRUCK. FOREMAN WAS LIFTING THE PIPE UP AND THE OPERATOR HAD RET URNED W/O THE KNOWLEDGE OF THE FOREMAN. ONE END OF THE PIPE FLIPPED UP APPARENTLY STRIKING THE OPERATOR KNOCKING HIM UNCONSCIOUS AND CUTS ON THEBACK OF HIS HEAD.

HE WAS CUTTING A PIECE OF SIDERAIL RUBBER IN THE SHOP WITH A UTILITY KNIFE AND IT SLIPPED AND CUT HIS THUMB. HE RECEIVED 6 STITCHES.
INJURED WAS TAKING THIS TRANSMISSION OUT OF AN R-50. HE WAS BREAKING A BOLT LOOSE, WITH A 3/4" SOCKET, AND BREAKER BAR. THE SOCKET BROKE CAUSING HIM TO STRAIN HIS BACK. HE WAS USING A PIECE OF PIPE ON THE BREAKER BAR.

EMP WAS PULLING SPREADER DOOR OPEN TO DISLODGE JAMMED STONE ON OUT #1 SCALPER SCREEN, HIS HAND WAS CAUGHT BETWEEN THE DOOR AND THE SCREEN. 2 FINGERS ON HIS RIGHT HAND WERE CUT LITTLE FINGER & NEXT FINGER THE FINGER NEXT TO HIS LITTLE FINGER REQUIRED 5 SUTURES
THE EMPLOYEE STARTED DOWN A RAMP WITH A EUCLID R-50 REAR DUMP. THE TRUCK STARTED TO FREEWHEEL AND WAS STEERED INTO A BUMPER PILE AT THE RAMPS END. AN AXLE BROKE WHICH PUT ADDED FORCE AGAINST T HE OPOSITE SIDE WHEEL LOCKS WHICH IN TURN GAVE WAY CAUSING THE TRUCK TO FREEWHEEL.
EMPLOYEE PICKED UP 40 LB WEIGHT FOR SCREEN SPREADER REACHED OUT 2 FT TO INSTALL THE WEIGHT WHILE THE SCREEN WAS RUNNING AND HUT HIS BACK.
EMPLOYEE WAS HELPING REPAIR A CRUSHER, USING A CHAIN TO HELP LIFT SHAFT SO IT COULD BE BLOCKED UP FOR REPAIRS AT SHOP
