EMPLOYEE STATED THAT WHILE DRIVING SHALE HAUL TRUCK HE FELT HIS BACK POP AND THAT HE WAS STRUCK BY TRUCK SEAT. HE STATED THAT HIS BACK HAD BEEN HURTING AND CONTINUED TO GET WORSE.
