Employee was exiting his truck at the scales to weigh. When he stepped down on the bottom step, it broke, throwing him backwards and he hit his head and shoulder on the platform. He passed out for about 3-4 minutes. Employee was taken to the ER by the supervisor. This resulted in a concussion and a shoulder contusion.

Employee was adjusting a round hay bale as it was being put in the mulcher. Used left arm to push on bale and arm became stuck in hay bale as it was moving, pulling arm, resulting in some pulled muscles.
