THE INJURED WAS DISMOUNTING LOADER TO OBTAIN TOOLS.HE SLIPPED AND FELL.WHEN HE HIT THE REAR BATTERY BOX IT CAUSED TWO FRACTURED RIBS ON HIS RIGHT SIDE.THERE WAS A LIGHT RAIN WHICH CAUSED THE T RAVELWAY TO BE SLICK.
AFTER CLIMBING ON GRADER TO LOOK AT HOUR METER I NAME SAID HE STEPPED OFF THE BACK OF THE GRADER WHEN HIS FOOT HIT THE GROUND I NAME SAID HE TWISTED HIS ANKLE WHICH RESULTED IN A SPRAINED ANKL E
AS HE WAS SITTING IN HIS TRUCK, A LARGE PIECE OF THE HIGHWALL FELL OFF AND STRUCK HIS TRUCK. THE TRUCK WAS ROCKED VIOLENTLY AND THIS CAUSED HIM TO STRAIN HIS BACK. (ALL OF ABOVE PER EMPLOYEE)
AS HE WALKED DOWN A HILL CARRYING A BALE OF HAT HE LOST HIS FOOTING WHEN HE SLIPPED ON A MUDDY ROCK.HE ROLLED APPROXIMATELY 30 FEET DOWN THE SLOPE.THIS FALL RESULTED IN A BRUISED BACK.

WHILE HELPING TO SPREAD HAY ON A SEEDED AREA, HE TRIED TO STOP A BALE OF HAY ROLLING DOWN THE HILL. THE BAIL OF HAY KNOCKED HIM OFF HIS FEET, AND HE STRAINED HIS BACK WHEN HE FELL TO THE GROUND.
