THE ROOF OF THIS MINE HAS A HISTORY OF LOOSE DDRAW ROCK. VISUAL INSPECTION WAS DONE BEFORE BOLTING THE AREA BEGAN, BUT THE ROOF WAS NOT TESTED BY SOUNDING. A PIECE OF ROCK BROKE LOOSE WHILE VICTIM WAS DRILLING A BOLT HOLE. THE ROCK STRUCK THE VICTIM IN THE FACE AND BODY, CUTTING HIS FACE AND LEG.
