Employee was pulling slack up on the miner cable. while working, a corner of a pillar rolled off striking him in the head back and leg.
EE was looking into SC reel and the lid closed and pushed his head into the reel. The EE received a cut on his eyebrow.

Employee was removing a piece of string from a bottom roller and his glove was caught by the roller and pulled his hand into the roller.
Employee was pulling a piece of cable over his shoulder and felt a sharp pain.
Employee was setting timbers & timber fell to the side and caught his right middle finger smashing it.
Employee was building a back stop for a belt head. Employee strained lower back while lifting a piece of sheet metal.

Employee was cutting the belt with a utility knife. The knife slipped out of the belt and cut his hand.
While raising the personal canopy, it swung and a rock fell, hitting the left thumb.
Employee was standing in the #5 entry, a rib rolled off, knocked him to the ground and mashed his little finger on the left hand.
Taking down a kenndy stopping and a piece of rock fell out and struck the employee across the nose
Employee was bolting top and his drill steel hit his left hand.
Fresh waterline came apart at a coupling and the end struck the employees knee.
Employee was laying on his back and lifting a driveshaft into place. As employee attempted to lift the shaft he felt a severe pain in his lower back.

Rib rolled off and struck employee in the back. He was working in #3 entry.

Changing scoop batteries on S&S scoop. Smashed & broke left ring finger & middle finger.
EE was holding plastic around hyd hose. Hyd oil was injected into index finger. (Did not receive this report of injury until 12/31/2008).
Lifted roofbolt to install in hole, felt pain in lower back.
Driving Mac 8 when front wheel hit a rock causing the steering wheel to spin around and hit left thumb
Operating scoop had hand laying on top of panel mashed finger between panel and frame.
Fall occurred approx 70 ft outby spad #457 in #7 entry. Approximate size of roof fall is 60 ft long, 18 ft wide, 5 ft thick.
Injured stated he was lifting a crib block, when he felt pain in groin area.
Sampling coal at Belt discharge. Large lump hit shovel and stoved right shoulder and wrists.
Walking and shuttle car cable caught legs and caused him to fall down.
Fall occured approx. 60ft in by spad #323 in the #2 entry approx size of fall is 4ft thick, 18ft wide and 50ft long.

Security office EE came out of parts trailer and was on his way to the port-potti when he tripped on a pallet and fell to the ground striking a piece of metal and cutting his right leg.
8/24/07, mine contracted our office to ask if our EE had notified us of an incident that happened the day before, he had not. EE stepped off roof bolter, twisted right ankle. Applied ice, sent to ER, referred to orthopedic DR. (8/27/07, EE returned our call and updated info). Cast to be placed on right ankle. Estimated time off is 6 weeks.
Hose busted on SkyTrak, hyd. penetrated ring finger on right hand.
