AT THE EVENING SHIFT CREW LEFT THE SECTION AT THE END OF THEIR SHIFT ON 8-20-93. THE LONG AIRDOX 3-WHEEL MANTRIP ENCOUNTED ROUGH ROADWAY. THE MANTRIP STRUCK A POT HOLE AND CAUSED THE EMPLOYEE TO BOUNCE FROM HIS SEAT. AS HE RETURNED TO THE SEAT, HE INJURED HIS LOW BACK.

THE EMPLOYEE WAS OPERATING A 21 S/C SHUTTLE CAR AND WAS TRAVELING FROM THE FEEDER CRUSHER TO THE CONTINUOUS MINER. AS HE APPROACHED THE MINER, THE SHUTTLE CAR ROOFED AND TORE DOWN A FLYBOARD. THE FLYBOARD SWUNG AROUND AND STRUCK THE OPERATOR IN THE HEAD.
