EE was helping to move the forks on a fork loader his hand was caught between the forks and the back board of the loader.

Walking off push beam in table, put his left hand on table for balance. When he stepped off push beam a piece of metal broke off in employee's left palm.
EE was putting the belly pan back on the miner when the lifting eye broke off letting the belly pan drop approx. 16" landing on the employees legs.

On oct 3rd, EE reported that on 9/20/07, while pulling on a cable he injured his back.
