A ROOF FALL OCCURED APPROX 500' OUTBY THE 001-0 WORKING SECTION IN A RETURN ENTRY. THE AREA HAD BEEN PREVIOUSLY DANGERED OFF. NO CLEANUP IS PLANNED AT THIS TIME.

EE WAS REMOVED WHILE IN PROCESS OF USING TORCHES TO CUT AND REMOVE AN OLD PAD LOCK THE METAL PARTS SUPPLY TRAILER INNER WALL INSULATION BECAME IGNITED BY FLAME OR HEAT FROM THE CUTTING TORCHES . THE INNER WALL CAUGHT FIRE AND ATTEMPTS WERE MADE TO EXTINGUIER WITH FIRE EXTINGUISHERS AND WATER UNTIL THE DISCUSSION WAS MADE TO LEFT BURN.
340 FOOT FROM THE FACE IN THE #1 ENTRY A 80' X 19' HEIGHT ESTIMATED TO BE 6' TO 12' HIGH FALL AT SPAD 55597 250 FEET INBY PORTAL OCCURRED. MSHA AND STATE WERE NOTIFIED AT 10:00 AM SAME DAY. WE WERE USING 5 FOOT RESIN BOLTS AT THE MINE.
