MINER WAS REMOVING RIGHT HAND CUT IN PILLAR RECOVERY. APPROX. 20' OF CUT HAD BEEN EXTRACTED. OPERATOR SAW ROOF BEGIN TO WORK NEAR BACK OF THE CUT HE BACKED THE MINER OUT AND SQUARED UP IN THE ENTRY. AS HE TRIED TO BACK ON OUT OF THE ENTRY, THE CUT FELL IN, CATCHING THE RIGHT SIDE OF THE RIPPER HEAD. APPROX. 15 MINS. LATER THE ENTRY FELL IN TRAPPING THE MINER.
A ROOF FALL STARTED INBY THE BREAKER TIMBERS IN THE OLD #1 ENTRY OF THE MAINS. IT APPEARS THAT AT THIS TIME IT FORCED A (HORSE BACK SLIP) LAYINGDOWN THE #1 ENTRY TO FALL ALSO. THIS FALL APPEAR ED TO BE 100 TO 120' LONG, 6-8 FEET HIGH AND 20'WIDE. THE CABLES ON MINER AND BOTH SHUTTLE CARS WERE CAUGHT BY THE FALL.
A PIECE OF DRAW ROCK FELL OUT OF ROOF IN BETWEEN ROOF BOLTS STRIKING EE IN BACK.
EMPLOYEE WAS KNOCKING KEEPERS OUT OF MINER TRAM CHAIN. A PIECE CHIPPED OFF THE HAMMER AND WENT INTO FINGER. REQUIRED STITCHES.
EE WAS PULLING MINER CABLE. HE HAD HIS KNEE ON A ROCK. HE SLIPPED AND INJURED KNEE. WORKED THRU 9-1-99. FIRST DAY MISSED 9-2-99. STILL OFF.
A ROOF FALL WAS FOUND AT #5 ENTRY ON MAINS APPROX 3500 FEET OUTBY 001 WORKING FACE AT SPAD #204. FALL WAS APPROX 100'LX20'WX5'T. VENTILATION OR EQUIPMENT NOT EFFECTED, ESCAPEWAYS NOT EFFECTE D. HIGH VOLTAGE CABLE UNDER THE FALL BUT NOT EFFECTED. FALL NOT EXPECTED TO BE CLEANED UP.
EE WAS SETTING STIFT JACK WITH CHAIN, CHAIN SLIPPED CAUSING IT TO FALL AND LAND ON EE S LEFT HAND-WAS APPROX. 6 INCHES IN DIAMETER AND 4 FT LONG.
EE SMELLED SMOKE, FOUND FEEDER ON FIRE IN SPAD #1275, LOADING POINT ON #1 SECTION. CAUSE OF FIRE UNKNOWN. FEEDER WAS ONLY THING EFFECTED. VENTILATION NOT EFFECTED BUT WAS SHORT CIRCUITED TO DI RECT SMOKE TO RETURN AIRWAY. FIRE FIGHTING BEGAN4:30AM AND EXTINGUISHED 5:30 -5:45AM. INSPECTOR MSHA ON SITE AT 6:25AM, OFFICIAL REPORT CALLED 8:45AM TO MSHA
EMPLOYEE WAS DRIVING A SCOOP. HE HIT A HOLE IN THE ROAD, CAUSING EMPLOYEE TO BOUNCE IN VEHICLE, WHICH CAUSED PAIN IN HIS LOWER BACK.
EE TOLD FOREMAN THAT HIS KNEE POPPED OUT OF JOINT 3 TIMES. FOREMAN ASKED EE IF HE GOT INJURED. EE STATED NO IT HAD DONE IT BEFORE AT HOME.

A ROOFFALL WAS FOUND AT APPROXIMATELY 9 AM BY BILL SALYERS IN THE FACE OF INACTIVE MAINS SECTION. FOUND AT THE LAST CROSSCUT FROM #5 ENTRY ACROSS TO #6 ENTRY, 240' IN BY SPAD #430 & #431. FA LL WAS APPROXIMATELY 20' WIDE, 70' LONG, 6-7' THICK. WILL NOT BE CLEANED UP.
HAPPENED BEFORE SHIFT. EE WAS COMING OUT OF CHANGE ROOM, WALKING DOWN THE STEPS AND TWISTED HIS ANKLE.
EE TWISTED KNEE LEFT AS HE STEPPED OFF THE LOADER. REPORTABLE BECAUSE HE COULD NOT RETURN TO WORK THE NEXT DAY.L
EE WAS UNPLUGGING BATTERY RECEPTICAL IN A LOW AREA AND WAS IN AN AWKWARD POSITION AND STRAINED HIS BACK. REPORTABLE BECAUSE EE WAS UNABLE TO RETURN TO WORK.
EE WAS OPERATING RAM CAR WHEN RAN OVER PIECE OF CRIB BLOCK IN ROADWAY (4X4) CAUSING HIM TO BOUNCEW SO HARD THAT HARD HAT CAME OFF AND NEARLY BOUNCED OUT OF SEAT.
EE WAS BOLTING TOP WHEN HIS LEFT HAND GLOVE CAUGHT BETWEEN STEEL AND HEAD OF PINNER TWISTING HANDS AND WRIST. REPORTABLE BECAUSE FRACTURE OF LEFT FOREFINGER.
EMPLOYEE WAS DRIVING FORKLIFT WHEN IT TURNED OVER AND HE TRIED TO CATCH HIMSELF WIHT HIS HAND A S FORKLIFT WAS TURNING OVER CAUSING LACERATIONS AND A FRACTURE TO HIS RIGHT FOREARM.
EE WAS UNLOADING SUPPLIES. ROFBOLTER OPERATOR SWUNG THE DRILL ARM OUT AND CAUGHT EE'S RIGHT KNEE AGAINST THE SCOOP.

EE WAS USING TWO PIECE SECTIONAL DRILL STEEL WHEN HIS FINGER GOT CAUGHT BETWEEN THE CONNECTING JOINT OF THE TWO PIECES AS HE WAS PUSHING STEEL INTO DRILL HOLE.
EE WAS HELPPING INSTALL A NEW SPEED REDUCER ON OUTSIDE BELTDRIVE. HE WAS LEANING OVER & TWISTING TO THE LEFT WHEN HE FELT SOMETHING POP IN HIS BACK.
EE WA BENDING BOLTS WHEN HE STRAINED HIS RIGHT SHOULDER. *ON RESTRICTED DUTY*
EMPLOYEE WAS STRAIGHTENING A ROOF BOLT TO INSTALL IT. HE EXPERINCED PAIN BELOW HIS RIGHT SHOULDER BLADE.
EE WAS ATTEMPTING TO GREASE THE SWIVAL PART OF THE SCOOP. THE GREASE GUN FITTING WAS DIFFICULT TO OPEN AND PRESSURE BUILT UP BY THE GREASE GUN RUPTURED THE HOSE WHERE THE EE WAS HOLDING IT, CA USING THE GREASE TO PENETRATE THE MIDDLE FINGER OF HIS LEFT HAND.
EE TWISTED RIGHT KNEE WHEN HE HUNG RIGHT FOOT UNDER THE PANIC BAR OF THE SCOOP AS HE WASS CRAWLING OUT OF THE SCOOP.
EE ALLEGES THAT AS HE WAS RUNNING SHUTTLE CAR IN BELT ENTRY OF 002 SECTION, CAR HIT A HOLE IN THE ROADWAY AND JARRED THE CAR. EE SAID HE FELT SOMETHING PULL IN HIS BACK. EE SAID HE WENT TO THE DR ON 6-16-97 AND AFTER EXAMINATION WAS TOLD HEHAD AN ACUTE LUMBAR STRAIN. EE DID NOT REPORT THIS ACCIDENT UNTIL 6-19-97. AFTER HE HAD BEEN TERMINATED.

EMPLOYEE WAS WORKING AT FEEDER AND WALKING THROUGH LOOSE COAL WHEN HE TWISTED HIS RIGHT KNEE.

EMP WAS ON KNEES AT WORK DOING PRE SHIFT WHEN HETURNED AROUND HE FELT A POP IN HIS KNEE AND HAD SEVERE PAIN.'
