HE WAS RUNNING THE BOLTING MACHINE. HE ENCOUNTERED SOME BAD DRAW ROCK AND HAD TO LEAVE THE HIT THE SLATE BAR CAUSING THIS TO PUNCTURE HIS UPPER LEFT THIGH THE EMPLOYEE RECEIVED STITCHES AND A TETANUS SHOT.
THE S&S 270 TRACTOR MALFUNCTIONED AND CAUGHT HIS ARM BETWEEN THE TRACTOR AND THE WALL OF THE MINE. HIS RIGHT ARM WAS CRUSHED ANDCUT OFF AT THE ELBOW.

WALKING BESIDE OF BARIDGE WHILE BARIDGE WAS MOVING, BRIDGE SHIFTED PINNING WORKMAN BETWEEN BARIDGE AND COAL RIB. RESULTED IN BROKEN ARM.
