Employee was helping to move the plant. Employee was unhooking chains off the beam and the loader backed up and hit the beam causing the beam to fall over and falling on employee's left leg causing a sprain to his left ankle and a contusion/strain to his left calf. He is restricted to sitting work only.

Had excessive amount of rain. The river went out of its banks and the pit area was flooded. There was no production on Monday 3-13 and Tuesday 3-14-06. The water started to recede and production started back up on Wednesday 3-15-06.

Employee was working on gates at the front entrance. He was using a hammer to drive a pin out of a hinge on the front gate. A piece of metal flew up and hit employee on the chin. The piece of metal went through his chin and chipped a lower tooth. Dermabond was used to close the laceration in his chin.

Plant had melting snow and 5 3/4" of rain the week of January 2 to the 8th. There was wide spread flooding in Noblesville. There was no damage to equipment of to any buildings. There were no injuries.

Employee was picking mud and rocks off the conveyor belt and when he pulled rocks, his shoulder popped. Employee received a dislocated right shoulder and is restricted.

Employee was under the fuel tank using a wrench to tighten a bolt on the vertical swing shaft on the dragline. Employee felt pain but did not report it until two days later. He thought he would be o.k. but had to go to the doctor. He recieved a right shoulder rotator cuff-Tendonitis. Employee is restricted.

An employee was operating the front-end loader (Kawasaki 115Z111) when it caught fire. Employee looked back and the loader was on fire. The fire was large and hot. The local Fire Department responded and extinguished it after 1 hour. There were no injuries. An ee reported the fire to MSHA via telephone on 9-29-04, 9:30 A.M.

AS MATERI AL WAS DUMPED ON EMPLOYEE, HIS HARD HAT WAS KNOCKED OFF AND HIS HEAD WAS CUT BY THE PIT RUN MATERIAL. EMPLOYEE RECEIVED STITCHES.
