Employee was using a garden hose to water down the area around the scales to keep the dust down. She stepped down a slight incline and slipped. She fractured her ankle when she slipped.

Employee was preparing to mount a cutting edge on a FEL. The cutting edges were shipped on a palet stacked three high. Employee was sliding the top cutting edge off of the stack. His finger was caught between the two cutting edges. The cutting edges weigh approx. 350 lbs. apiece. Employee sustained a cut that required stitched and a broken finger. Both were to the same finger.

EMPLOYEE WAS OPERATING SCRAPPER. HIS BACK STARTED HURTING ON 1/9/91 IT CONTINUED TO GET WORSE. HE FINALLY REPORTED THIS TO HIS SUPERVISOR ON 1/15/90. EMPLOYEE HAS SUFFERED A DISK INJURY.
