Employee was attempting to lift a tire with the truck hoist. He unhooked the hook from the truck and was holding it out to clear the truck. He accidently engaged the hoist while still holding the hook and caught his finger between the hook and the truck. He fractued his finger and needed surgery to repair torn tendons.

Injured employee was pulling on a piece of angle iron to straigten it out. The angle iron broke at the weld causing him to fall on his ankle. Will require surgery.

The employee was unhooking a chain that was used to pull a stuck haul truck out of the mud with a front-end loader. The loader operator started to pull away. The employee motioned for him to stop. The loader was stopped abruptly which caused the bucket to dip. The employee's foot was under the bucket when it dipped. The employee sustained broken bones in three toes.
