EMPLOYEE WAS WALKING DOWN A TRECCH STEP THE WRONG WAY AND CHIPPED HIS HIP BONE
PILING ROLLED ON TOP OF HIS RIGHT FOOT. SAFETY STEEL TOE SHOES WERE BEING WORN AT TIME OF ACCIDENT
EPLOYEE SLIPPED OFF A BOARD AND HAD A SLIGHT ABRASION
EMPLOYEE WASOPERATING BHM EQUIPMENT INSTALLING 2 1/2" CUTTING JET SECTIONS. HE HAD PLACED A HOLDING CLAMP AT THE TOP OF ONE OF THE SECTIONS BUT DID NOT SECURE IT. WHEN THE SECRTION WAS LIFTED THE CLAMP SLID DOWN AND HIT HIS LEFT HAND, INJURING HIS FINGER.
EMPLOYEE ACCIDENTLY STRUCK WELDING ROD ON BACKHOE BUCKET AFTER HE HAD RAISED HIS WELDING HOOD CAUSING WELDING FLASH TO EYES
EMPLOYEE LEFT HAND WAS LOCATED ON CHAIS OF DRILL RIG. THE DRILL RIG FOOTSTAND WAS PARTIALLY RAISED TO ALLOW DRAIN PAN MOVEMENT. AFTER PAN WAS MOVED. THE FOOTSTAND AUTOMTICALLY FELL TO ITS INIT IAL POSITION AND MASHED RING FINGER ON LEFT HAND.
EMPLOYEE WAS USING BUSH AX TO CUT SMALL BRUSH WHEN ONE SMALL TREE HIT LEFT LEG.
