Broken Hydraulic line sprayed excavator bucket, victim did not realize and stepped on bucket, slipped on oil spray and fell into bucket. Landed on shoulder tearing rotator cuff.

(See Attached) EE loading powder truck with 50 lb. bags of ANFO. EE stated he had aggravated his lower back and needed to go home and lay down. EE RTW 7/3 and worked full shift. We did not work on 7/4. EE came in 7/5 prepared his truck and started work. At 7:30 a.m. EE came to my office and stated he would make a doctor's appt. because his back was still painful.

He was climbing onto the passenger side of the terex truck in order to ride to the company lunch room. As he was climbing he experienced some pain after reporting to me he left work and went to physician. Was diagnosed with ruptured bicep and prescribed meds. Was instructed to rest his arm until 6/7/10. Still suffering discomfort he will be seen by his physician.

EE and other ee were working in an enclosed 60'x80' shop using the forklift to change tires on a pit truck. They advised me they had headach, neausea, fatigue, & shortness of breathe. Both were transported to the hospital. ER determined they had mild carbon monoxide poisoning. Both were released from the ER. A CO monitor was used to determine the forklift was the cause and repair

EE was using a shovel to remove partially frozen Ag lime that was stuck in the bucket of the loader. He believes he strained his back at some time in this process. He did not notice any pain at this time. He had pain when he got up in the morning of 2/8/07. He came to work and advised that he couldn't climb in out of the loader and was going to see his personal physician.

INJURED & 2 OTHER EMPLOYEES WERE DISMOUNTING AN 1800 X 33 TIRE FROM FRONT AXLE OF TRUCK. HE INSTALLED HYDRAULIC BEAD BREAKER ON THE TRUCK SIDE OF THE WHEEL TO PUSH TIRE OFF OF RIM. WHEN PRESSURE WAS APPLIED TO BREAKER IT CAME OFF THE WHEEL. HIS HAND WAS ON THE BREAKER WHEN IT CAME LOOSE AND WAS MOMENTARILY STRUCK BETWEEN THE TRUCK FRAME AND THE BREAKER.

HE APPLIED PARKING BRAKE AND LEFT TRUCK, THE TRUCK BEGAN TO ROLL BACKWARDS, HE ATTEMPTED TO MOUNT TRUCK TO STOP IT. THE TRUCK JACKNIFED AND HE FELL BETWEEN TRACTOR CAB AND THE TRAILER WHERE HIS ARM WAS PINCHED BETWEEN CAB AND TRAILER.

HE FELT A "POP" IN THE JOINT AND SAT DOWN AT THE BOTTOM OF THE STAIRS.
