EE was using a board as a pry to loosen a turbo from a piece of equipment. This lead to him tearing his rotator cuff.

He was using a cutting torch, he had on a t-shrit and a sweat shirt. The line on the torch got a hole in it and the gas went up into his shirt. When a spark hit the gas that was then trapped in his shirt, it caught on fire, therefore burning EE. He was taken to the hospital, then trasported to UC hospital burn center for 3rd and 2nd degree burns.

WAS HELPING PUT A SHAFT IN A SCREEN SHAKER IN SHOP. WAS HOLDING IT UP ON THE OTHER SIDE. TWO OTHER EMPLOYEES WERE PUSHING IT FROM THE OTHER SIDE. THE BAR SLIPPED AND RIGHT THUMB WAS CAUGHT AND INJURED BETWEEN SHAFT AND SCREEN SHAKER.
EMPLOYEE WAS FUELING LOADER, MISSED STEP AND FELL TO FROZEN GROUND.
