There was very hot temperatures in West Texas on this day. The temperature was 103 degrees at time of the incident. Employees Air conditioner was not properly working throughout the day in the Haul truck. The heat caused the employer to pull over on the side of the haul road, where 30 minutes later EE was found passed out from heat.

Employee was utilizing a grinder to cut a bearing sleeve. This task was necessary to replace the bearing on the head pulley of the C1 belt. Midway through the task, the grinder unexpectedly kicked back towards the employee, causing the grinder's disk to make contact with the employee's left forearm resulting in a laceration.

Individual was greasing excavator prior to working and outdoor conditions were windy and dry. EE recalls sand blowing into right eye. EE attempted to wipe it away and believed EE had done so. Reported to manager on 28 Aug as EE felt EE needed to get medical attention to the eye.
IW was struck by co-workers hardhat. Hardhat was swung down and away and was swung into the injured worker standing behind.

While replacing a damaged digger tooth on excavator bucket, as hammer was swung it impacted corner of tooth and broke off and entered right hand between thumb and index finger (Crotch of hand).
While climbing down the ladder, IP right foot slipped off the last step (rung) on the ladder. IP attempted to catch self resulting in a strain to EE's right elbow. Incident occurred on 06/28/22, reported on 06/29/2022 First Aid Case. Seen by MD on 07/06/2022.
Employee fell on frozen ground landing on their side resulting in a shoulder strain.
