Operating a grader when a hydraulic hose blew, causing the operator to lose control of the machine. Operator jumped from the machine, fracturing their right leg/ankle.

Operator was operating a D11N tractor and stopped machine for lunch break. Operator stated was attempting to exit the machine when EE lost balance and fell from the operator's platform to the ground below. Operator fractured right hip, tailbone and injured right elbow.

Running oil through a wire wheel and a piece of the wire came out and went into my right eye. The wire was sticking out of the eye. Miner was transported to a local hospital where surgery was performed to remove the metal.

Loading rock with a 992G loader when the loader apparently struck an undetonated primer. The concussion caused an air blast that shattered two windows in the loader and the back window of a 777F truck. There were no injuries to miners.

Employee was walking from the parts trailer with the Ohio CAT mechanic when he suffered a possible stroke and fell to the ground. Originally it was believed to be related to diabetes but the medical crew checked his sugar level and it was acceptable. We will not know the cause until he receives treatment.

Miner was tossing 30 pound bags on the blast site when he felt a pain in left shoulder. Did not have any further pain so he did not report the incident until 11/5/2013 to Hilltop Energy. He said he awoke on 11/5/2013 with pain in shoulder went to doctor.

VICTIM WAS FOLLOWING ANOTHER TRUCK WHEN THE VICTIMS TRUCK HIT A PATCH OF ICE AND HE SLID EVENTUAL LY STRIKING BERM THE VICTIM WS NOT WEARING HIS SEAT BELT AT TIME. THE VICTIM HAD A STRUCTURE OF THE L1 VERTEBRAE AND IS TO REST.

IT FELL BETWEEN THE SCREEN AND THE HIGHWALL AND STRUCK THE VICTIM IN THE THIGHS OF BOTH LEGS JUST ABOVE THE KNEES. THE VICTIM SUFFERED BRUUISING OF BOTH LEGS AND ONE SMALL CUT. HE WAS SENT FROM THE HOSPITAL THE SAME DAY.

VICTIM WAS LAYING ON BACK LOOSENING CUTTING EDGEBOLTS ON A SCRAPER. HE WAS USING AN AIR IMPACT WRENCH WHICH HE WAS HOLDING IN LEFT HAND AND USING RIGHT HAND TO SUPPORT SOCKET. AS HE TRIGGERED WRENCH, SOCKET CAUGHT AND CAME LOOSE. WHEN THIS HAPPENED, WRENCH CAME BACK AND STRUCK VICTIM IN LEFT CHEEK. THE RESULT WAS A 3/4" CUT. VICTIM WAS TAKEN TO DOCTOR AND WOUND STITCHED.

EE HAD PARKED HIS MACHINE AFTER FINISHING LOADING TRUCKS. HE WAS SITTING IN THE CAB, WITH ENGINE RUNNING, FILLING OUT HIS LOADING FORM WHEN FIRE ERUPTED FROM UNDER HIS SEAT. EE IMMEDIATELY JUM PED FROM MACHINE. EE NOTICED OIL ON FLOORBOARD AND REVVED HIS ENGINE SLIGHTLY THAT WAS WHEN FIR ERUPTED.

BECAUSE OF FLAT ANGLE OF PENETRATION, T HE METAL DID NOT STRIKE ANY ORGANS AND WAS EMBEDDED BENEATH SKIN. IT WAS SURGICALLY REMOVED AND STITCHED (3 SUTURES).

59339M WAS USING A SLEDGE TO HAMMER ONT HE RIPPER TOOTH OF HIS MACHINE WHEN A SLIVER OF METAL STRUCK AND PENETRATED HIS RIGHT UPPER CHEST. THE HOSPITAL DETERMINED THAT THE SLIVER DID NOT CAUS 50158 SERIOUS INJURY AND POSED NO FURTHER THREAT. AFTER WOUND WAS CLEANED AND DRESSED THE VICTIM WAS RELEASED WITHOUT RESTRICTION.

EE WAS USING A TORCH TO REMOVE A STRIP OF METAL FROM THE BED OF A EUCLID 5-50 ROCK TRUCK. AS EE MADE HIS FINAL CUT THE METAL STRIP FLIPPED UP AND STRUCK HIM IN THE LOWER LIP. EE STATED HE HAD MISSED CUTTING THROUGH A WELD WHICH KEPT THE STRIP UNDER TENSION CAUSING THE METAL TO FLIP.

VICTIM SAID IT HAPPENED FA ST SO HE WASNT SURE BUT FELT WHEN HE DUCKED HIS HARDHAT CAME OFF AND SMALL PIECE STRUCK HIM NEAR HAIRLINE.
