EE HAULED FELDSPAR IN CO TRUCK. AT THE END OF THE HAUL, THE BOX ON THE TRUCK NEEDED TO BE CLEANED. IN THE PROCESS OF CLEANING THE BOX WITH THE AIR HOSE, A PIECE OF FOREIGN MATERIAL EMBEDDED IT SELF IN HIS RIGHT EYE. ON 9/7/98 HE WENT TO THE URGENT CARE CLINIC IN CUSTER AND HAD IT REMOVED. HE WAS OFF WORK 9/7/98 ABD RETURNED TO WIRK ON 9/8/98.

HIS RIGHT HAND WAS BROKEN AND RECEIVED LA CERATIONS. EMPLOYEE IS NOT A WAGNER MINING EMPLOYEE, BUT A PACER CORPORATION EMPLOYEE. EMPLOYEE HAD SURGERY ON HAND ON 8-3-94 AT RAPID CITY REGIONAL HOSPITAL.
