EE HAULED FELDSPAR IN CO TRUCK. AT THE END OF THE HAUL, THE BOX ON THE TRUCK NEEDED TO BE CLEANED. IN THE PROCESS OF CLEANING THE BOX WITH THE AIR HOSE, A PIECE OF FOREIGN MATERIAL EMBEDDED IT SELF IN HIS RIGHT EYE. ON 9/7/98 HE WENT TO THE URGENT CARE CLINIC IN CUSTER AND HAD IT REMOVED. HE WAS OFF WORK 9/7/98 ABD RETURNED TO WIRK ON 9/8/98.

EE WAS WELDING A GUARD ON THE PICKING BELT AT THE ST. LOUIS MINE, A FLASH BACK OCCURRED APPARENTLY FROM THE SNOW AND HE RECEIVED WELDING FLASH IN HIS EYES. EE WENT TO CLINIC FOR CHECKUP AND RE CEIVED A PRESCRIPTION. NO LOST TIME. HE RETURNEDTO WORK ON HIS NEXT SCHEDULED WORK DAY.

EMPLOYEE WAS WELDING A HOPPER ON THE PICKING BELT A THE ST LOUIS MINE.THE WELDING HOOD BOUNCED BACK JUST A LITTLE AND A SPARK INSIDE THE HOOD AND BURNED THE CORNEAS OF BOTH EYES.EMPLOYEE WORKE D ALL DAY.EYES STARTED BOTHERING HIM FRIDAY EVENING.HE WENT TO THE EMERGENCY ROOM AT CUSTER COMMUNITY HOSPITAL WHERE THEY CHECKED HIS EYES AND GAVE HIM SOME EYE DROPS.THE RE-CHECK ON SATURDAY

EMPLOYEE LIFTED THE HOSE FROM THE SIDE OF THE ENGINE, ABOVE THE FAN BELTS, THAT WERE OBSTRUCTING HIS VIEW. HIS RIGHT HAND WAS STRUCK BY THE FAN BLADE. HIS RIGHT HAND WAS BROKEN AND RECEIVED LA CERATIONS. EMPLOYEE IS NOT A WAGNER MINING EMPLOYEE, BUT A PACER CORPORATION EMPLOYEE. EMPLOYEE HAD SURGERY ON HAND ON 8-3-94 AT RAPID CITY REGIONAL HOSPITAL.

EE WAS PERFORMING MAINTENANCE ON A 75B MICHIGAN LOADER. THE SCREWDRIVER SLIPPED AND HE PUNCTURED HIS LEFT HAND. EE WENT TO THE FAMILY HEALTH CARE CLINICC, SAW DR GRABER AND GOT A TETNAS SHOT. THERE WAS NO LOST TIME ON THIS INJURY.
