THE ROOF FALL OCCURRED ON 1ST LEFT SECTION. RETREAT MINING WAS IN PROGRESS. THE FALL WAS APPROX. 110 FT LONG AND 8' IN HEIGHT. THE FALL OCCURRED IN #2 ENTRY- 2 BLOCKS INBY THE SECTION BELT TAI LPIECE.

INDIVIDUAL WAS ROOF BOLTING HE WAS STANDING WITH HIS RIGHT FOOT. RESTING ON A SMALL ROCK. HIS FOOT SLIPPED FROM THE ROCK AND STRUCK THE FRONT OF THE BOLT MACHINE. HE BRUISED THE SMALL TOE ON H IS FOOT. ON 6-24-94. HE RETURNED TO WORK BUT LEFT EARLY BECAUSE HE FELT ILL. HE WAS SEEN BY DR LATER ON 6-24 AND HOSPILIGHTED FOR AN INFECTION. WE DID NOT RECEIVED THE WORKERS COMPHENSATION R
