PAULDING QUARRY Metal/Non-Metal

Employee was removing rock from the bottom side of the #4 conveyor. There were employees digging metal from a loader shank out of the crusher and dropping pieces onto the catwalk below. A softball-sized piece bounced off of the catwalk, fell to the level below and struck employee on the right shoulder blade, causing a small fracture to his scapula.

Utility knife being used to cut conveyor belting slipped causing a small laceration to the left leg requiring two stitches.

Picking up a box of orange boosters, felt something pop in left side of back.

Ee was under truck removing drive shaft to repair fuel pump in gas tank. Parking brake was engaged but when drive shaft was removed the parking brake released. With the tires not being chocked and parked on a slight grade, the truck began to roll back. Ee is scheduled for consultation on 8/3/04 to have small bruise drained.

EE'S FINGER WAS CUT WHILE OFF LOADING SCREENS.

EMPLOYEE WAS STEMMING HOLES AND SHOVELING AND HIS LEFT FOOT SLIPPED FROM ROCK MOVEMENT AND SPRAINED HIS LEFT KNEE.

EE WAS OPENING & CLOSING A POCKET KNIFE (NOT PERFORMING A JOB FUNCTION AT THE TIME.) THE KNIFE CLOSED AND CUT HIS RIGHT HAND POINTER FINGER. THE CUT REQUIRED 4 STITCHES.

EE WAS ATTEMPTING TO LIFT BLOCKS WHILE IN A SEATED POSITION. EE COMPLAINED OF SORENESS AND STIFFNESS TO LOWER BACK.

LARGE ROCK DISLODGED FROM 25' EMBANKMENT WHERE EMPLOYEE WAS WORKING BELOW. ROCK ROLLED DOWN THE HILL STRIKING EMPLOYEE IN THE LOWER BACK CAUSING BRUISES AND ABRAISIONS. EMPLOYEE RETURNED TO WORK THE NEXT DAY FOR LIGHT DUTY.

EE WAS USING A TORCH TO CUT STEP FRAME TO REPLACE WITH NEW STEP. AS HE WAS CUTTING, THE SLAG WAS BOUNCING OFF OF THE SIDE OF STEP FRAME, IN DOING SO IT CAUGHT HIS COVERALL'S ON FIRE, BURNING H IS LEG ON THE LEFT CALF.

EE WAS HELPING REMOVE BUCKET FROM 330 BACKHOE. HE WAS RESTING A 30 LB STEEL BAR ON THE BUCKET WHEN THE BUCKET SHIFTED FORCING THE BAR INTO EE'S NOSE.
