Martin Marietta Materials, Inc · Struck by falling object

Employee was removing rock from the bottom side of the #4 conveyor. There were employees digging metal from a loader shank out of the crusher and dropping pieces onto the catwalk below. A softball-sized piece bounced off of the catwalk, fell to the level below and struck employee on the right shoulder blade, causing a small fracture to his scapula.

Florida Rock Industries, Inc. · Caught in, under or between a moving and a stationary object

Ee was under truck removing drive shaft to repair fuel pump in gas tank. Parking brake was engaged but when drive shaft was removed the parking brake released. With the tires not being chocked and parked on a slight grade, the truck began to roll back. Ee is scheduled for consultation on 8/3/04 to have small bruise drained.

LARGE ROCK DISLODGED FROM 25' EMBANKMENT WHERE EMPLOYEE WAS WORKING BELOW. ROCK ROLLED DOWN THE HILL STRIKING EMPLOYEE IN THE LOWER BACK CAUSING BRUISES AND ABRAISIONS. EMPLOYEE RETURNED TO WORK THE NEXT DAY FOR LIGHT DUTY.
