On December 12, 2009 at approximately 6:38 am, a pre-dawn, low temperature accident occurred on a haulage road at PE Southern Pike Co. The driver of DT 178 exited the loaded coal truck, resulting in his death.

EMPLOYEE WAS OPERATING A 320 LOADER NAD CUT HTE ENGINE TO WAIT FOR A BLAST TO BE PUT OFF, AND WHEN HE STARTED TO START THE ENGINE AGAIN, HE LOADER STARTED ROLLING DOWN THE HILL AND THE EMPLOY EE PANICKED. HE DID NOTHAVE HIS PARK BRAKE SET, AND THE LOADER WAS STILL IN GEAR. THIS IS WHY IT DIDN'T START IMMEDIATELY.
