As employee was walking, his feet slipped out from underneath him, causing him to fall and land with his right leg pinned underneath him, resulting in a fracture to his fibula.

Got out of tractor/truck to clean windshield. Lifted hood on tractor. Climbed up on diver side tire to clean the windshield, foot slipped. Fell between the hood & bumper, stuck left rib cage.

EMPLOYEE WAS OPERATING A 320 LOADER NAD CUT HTE ENGINE TO WAIT FOR A BLAST TO BE PUT OFF, AND WHEN HE STARTED TO START THE ENGINE AGAIN, HE LOADER STARTED ROLLING DOWN THE HILL AND THE EMPLOY EE PANICKED. HE DID NOTHAVE HIS PARK BRAKE SET, AND THE LOADER WAS STILL IN GEAR. THIS IS WHY IT DIDN'T START IMMEDIATELY.
SLID DOWN CATWALK CUT HIS ARM N PIECE OF METAL STICKING UP ON THE CATWALK CUT HIS RT ARM GETTING APPROX SEVEN STITCHES
EMPLOYEE SLIPPED ON ICE WHILE WORKING ON A 992 CAT LOADER. PULLED MUSCLE IN RIGHT LEG WHEN HE SLIPPED AND FELL ON THE DECK PART OF THE MACHINE.
