Hauler driver of semi-dump unit dumping limestone onto ground at processing plant. Semi-dump unit rolled onto its side.

On road tractor trailer driver was dumping the limestone from the quarry at the processing plant and the tractor trailer rolled 90 deg. on its side.

Our company mechanic was doing some repairs on our Mechanical Scaler when he turned to dismount machine and lost his footing, falling 4 feet to the mine floor. The accident resulted in an emergency room visit where he received 9 stitches in his right ring finger and an xray to arm and shoulder, then released to restricted duty for 10 days.

The double boom drill was drilling a crosscut from 1E13 to 2E1, when the left drill steel came in contact with some undetected explosives in a bootleg. The charge detonated and damaged the stinger cylinder, knocked the hammer off the rail and shattered the front windshield.

THE SLINGER TRUCK WAS STUCK ON A RAMP AND WAS BEING PULLED OUT, WHEN THE MATERIAL SLID UNDER THE RIGHT SIDE REAR WHEELS CAUSING IT TO TIP OVER AND THE OPERATOR WAS TOSSED ACROSS THE CAB. THE O PERATOR RECEIVED MEDICAL TREATMENT FOR A CONTUSION AND CONCUSSION.
