A haul truck was in use in a sand quarry when a fire started in the engine compartment. The operator immediately shut the truck off, exited the cab, dismounted the truck, and distanced self safely from the machine. The fire was extinguished by local EMS. The extensive damage to the front-end of the truck is expected to result in a complete loss of the machine.

A Relief Operator while closing a valve in the crushing area felt a pop in left elbow. Employee immediately reported the occurrence to supervisor. As the shift progressed the pain, motion, and hand grip strength worsened causing the employee to request a medical evaluation.

BUCKET ELEVATOR BELT BROKE AND FELL. A CRANE WAS USED TO LIFT THE BELTING AND RE-LOOP IT OVER THE HEAD PULLEY INTO HOUSING. EMPLOYEE DIRECTING THE CRANE USED NYLON STRAP AS A CHOKER. WHEN THE BELT HAD BEEN LIFTED 30 FT THE NYLON STRAP PINCHED THROUGH BELT CAUSING IT TO FALL. THE EMPLOYEE GUIDING THE BELT WAS STRUCK IN THE HARD HAT AND BACK BY THE BELTING AND BUCKETS.

THE EMPLOYEE REACHED UNDER THE TANK TO REMOVE AN OBJECT FROM THE FOUNDATION AND WHEN HE DID SO, THE CRANE "LEAKED OFF" AND LOWERED THE TANK ENOUGH TO PIN THE EMPLOYEE'S ARM FOR A FEW SECONDS.
