EE WAS INSPECTING FIRE EXTINGUISHERS ON PL-5 (988 A). REMOVED EXTINGUISHER FROM BEHIND SEAT PLACED EXTING. ON OPERATORS SEAT LOOKED AT GAUGE ON EXTING. TOOK INK PEN OUT OF SHIRT POCKET TO DATE TAG ON EXTING. IT FELL TO THE FLOOR, DISCHARGINGINTO EYES, SINUS PASSAGE, THROAT, MOUTH & LUNGS.

INJURED WAS CLEANING BOTTOM ROLLERS ON A STATIONARY BELT WHEN THE 12' METAL BAR HE WAS USING BECAME WEDGED BETWEEN THE BELTING & THE BOTTOM ROLLER. THIS CAUSED THE BAR TO STRIKE UNDER HIS LEFT ARM.
