EE WAS INSPECTING FIRE EXTINGUISHERS ON PL-5 (988 A). REMOVED EXTINGUISHER FROM BEHIND SEAT PLACED EXTING. ON OPERATORS SEAT LOOKED AT GAUGE ON EXTING. TOOK INK PEN OUT OF SHIRT POCKET TO DATE TAG ON EXTING. IT FELL TO THE FLOOR, DISCHARGINGINTO EYES, SINUS PASSAGE, THROAT, MOUTH & LUNGS.
EE WAS WALKING FROM CONVEYOR TO TOOL TRAILER WHEN HE STEPPED IN A HOLE AND FELL WHICH CAUSED HIM TO TWIST HIS RIGHT KNEE.
EMPLOYEE WAS HOOKING COME-A-LONG TO 489 CONE CRUSHER IN ORDER OT ADJUST IT. HE WAS STANDING ON CRUSHER WHEN HE SLIPEPD AND FELL APPROX 4' LANDING IN A SITTING POSITION, ON THE MAIN FRAM OF TH E CRUSHER.
INJURED WAS WORKING ON FLAT DECK SCREEN. A PIECE OF ANGLE IRON WAS SECURED TO SCREEN STRUCTURE--WHEN BELT WAS STARTED FOR TESTING THE STRAP BROKE ALLOWING ANGLE IRON TO HIT THE BELT AND BOUNCE BACK STRIKING HIM IN THE LEG.

INJURED WAS CLEANING BOTTOM ROLLERS ON A STATIONARY BELT WHEN THE 12' METAL BAR HE WAS USING BECAME WEDGED BETWEEN THE BELTING & THE BOTTOM ROLLER. THIS CAUSED THE BAR TO STRIKE UNDER HIS LEFT ARM.
