DRIVER LOST CONTROL OF TRUCK WHILE CHANGING GEAR. HE THEN JUMPED FROM THE TRUCK STRIKING HIS HEAD OVER A ROCK CAUSING THE FATAL INJURY ACCORDING TO THE ONLY WITNESS.

EE WAS ASSISTING INDISASSEMBLING EQUIPMENT IN A MINE THAT THE CO. WAS NO LONGER OPERATING. HE WAS ON TOP OF A SHOVEL TAKING THE GANTRY LOOSE, WHEN AN ARM ON THE GANTRY SWUNG FREE KNOCKING HIM OFF THE MACHINE. EE LANDED ON HIS BACK.

EE WAS DUMPING A LOAD OF STONE WHEN THE SPREADER CHAINS CAUGHT IN THE TAILGATE CAUSING THE LOAD TO SHIFT AND RAISING THE TRUCKS FRONT UP. AT THIS TIME THE SPREADER CHAINS BROKE CAUSING THE FRO NT OF THE TRUCK TO FALL TO THE GROUND. HE WAS STAKEN UP INSIDE THE TRUCK.
