This was not an employee. This was an emergency rescue worker onsite to save a dog. I do not have the correct DOB or SS#. A loose rock fell and struck rescue worker when the rescue worker was rappelling down the side of the high wall to save a dog.

Employee was helping to change blow bars on the impact crusher. Left wrist was caught between the blow bar and the blow bar holder tool. Employee was diagnosed with a left wrist contusion/bruise. During follow up appointment on 07/13/2016 EE was referred to physical therapy by the treating physician's assistant.

While getting off John Deere loader that had been parked at the shop in a well lilt area, he began to dismount the loader and put one foot on the top rung and went to step down with the other foot, and lost his grip on the hand rail with his left hand. He swung around, but couldn't hold himself. He fractured his right pelvis. Should return to work in 4-6 weeks, per his doctor.

Employee took his yard loader to Shop service area to add hydraulic oil. He raised hood to put oil in reservoir. Wind was blowing as it was starting to rain. His left arm was resting, with left hand on hood latch. Wind caught hood and slammed it onto his hand. The latch pierced his hand between his index and middle finger.

Employee was using a 16 lb. sledge hammer to knock out a lock that holds in the impactor bars. He missed the lock and hit the impactor and the hammer bounced and hit his right foot behind the steel toe portion of his shoe.

Employee was digging into hard rock area in the pit. Loader was on slight incline when bucket slipped off of rock and employee pulled a muscle and felt pain in right lower back area.
Employee came out of the electrical building and was walking down the steps when he stumbled and fell on concrete below. He fell onto his right wrist. At the time of this incident he did not complain of any injury; although the fact that he had stumbled and fallen was noted.

EE WAS IN A SITTING POSITION, WELDING A HANDRAIL ON A STAIR PLATFORM. HIS BACK WAS TO THE STAIRS. ANOTHER EE WAS ON THE PLATFORM ABOVE. THE EE ON THE HIGHER PLATFORM KICKED THE ANGLE IRON WHICH FELL AND HIT THE EE ON THE BACK OF THE HEAD.
