DURING A NORMAL CONE ADJUSTMENT, EE'S INDEX FINGER & THUMB ON R HAND WERE SMASHED BETWEEN THE UPPER ASSEMBLY & SHIM STACK. LACK OF COMMUNICATION CONTRIBUTED TO THIS ACCIDENT! 3 PEOPLE, PLANT O PERATOR, PLANT OILER, & TRUCK DRIVER WERE INVOLVED IN THE ADJUSTMENT OF THE CONE.
