Employee was grinding rivets on a belt splice, high wind condtions, outdoors. Wind blew small metal fragment into right eye. Employee washed out eye and continued work. Eye remained irritated. Sought medical attention on 8/4/9

Worker was performing servicing operations on a portable crusher. Work requires considerable bending at the waist. Upon straightening from a flexed position, worker noticed sharp pain at lower back and down left leg. Worker has experienced pinched nerve in this area in the past.
