EE was sitting in truck #7694 when getting loaded ee twisted around to look at the operator when ee felt something pull in my back.

EE WAS MOVING (ROLLING) A CAT 980C RIM INTO THE BUCKET OF ANOTHER 980 LOADER HE WENT TO GET A BETTER GRIP, KNELT DONW, & HIS RIGHT KNEE POPPED
EE WAS ATTEMPTING TO MOVE SOME LOSOE AND PACKED DIRT FROM A STEEP GYPSUM LEDGE OT CLEAR A PLACE TO SET THE DRILL MAST. HE WAS USING HIS RIGHT FOOT TO KICK THE DIRT OUT OF THE WAY. ALSO USED THE SIDE OF HIS FOOT AND IN DOING SO, TWISTED HIS RIGHT KNEE. EE DID NOT SEEK MEDICAL CARE UNTIL 2-24-99.

THE DRILL STEEL WAS STUCK. EE WAS NOT ABLE TO REMOVE THE STEEL WITH THE DRILLING EQUIPMENT, SO HE MANUALLY TRIED TO REMOVE THE STEEL. HE WAS DIRECTING ANOTHER PERSON TO MOVE THE ROLLERS INWARD AND HIS THUMB WAS PINCHED IN BETWEEN THE ROLLER EQUIPMENT AND THE STEEL.

A LARGE ROCK WOULD NOT GO THROUGH THE JAW CRUSHER. EE WAS PUTTING A CHAIN AROUND THE ROCK SO THEY COULD PULL IT OUT WITH THE LOADER. HE GOT HIS RIGHT THUMB, INDEX FINGER AND MIDDLE FINGER CR USHED BETWEEN THE ROCK AND THE JAW.
