EMPLOYEE WAS CLEANING UP AROUND CONVEYOR WHEN A ROCK BOUNCED ON THE CONVEYOR AND FELL OFF. IT HIT THE EMPLOYEE ON THE RIGHT SHOULDER BLADE.

WHILE CHANGING SCREENS ANOTHER EMPLOYEE HIT THE SCREEN DECK WITH A LOADER CAUSING HIM TO LOSE HIS BALANCE AND SCRATCH HIS LEG. A WEEK LATER THE SCRATCH BECAME SEVERELY INFECTED AND REQUIRED ME DICAL ATTENTION. WORK LOSS TIME STARTED 6/14/98WHEN EMPLOYEE REPORTED INFECTION.
