Rogers Construction, Inc · Fall to the walkway or working surface

Tripped over a rock or the fuel hose, EE was not sure which one. Landed on right knee on frozen gravel. EE was limping and scratched knee. EE immediately cleaned with antiseptic and covered the wound.

Rogers Construction, Inc · Caught in, under or between a moving and a stationary object

Foreman and operator adjusting hydraulic controls on crusher, involves operator to run hydraulic control to lift adjusting nut. Foreman had hand placed under adjusting nut, when operator lifted hydraulic ram to loosen nut. Foreman's hand in an unsafe position, resulting in 4 fingertips on left hand to be crushed and lacerated.
