On 2 Feb 12, employee was attempting to repair equipment. Employee felt something abnormal in his back while trying to remove a nut. Employee did not report the incident to his supervisor until 10 Feb 12. On 11 Feb 12, employee went a clinic and received medication. On 17 Feb 12, employee was taken to Concentra who placed the employee on restriction and medication.
