I.NAME WAS PULLING ON THE AIR HOSE TO TAKE UP SLACK IN IT WHEN ANOTHER EMPLOYEE PICKED UP THE AIR HOSE TO ASSIST HIM. THIS SHIFTED THE WEIGHT OF THE HOSE THAT I.NAME WAS PULLING ON, AND CAUSED HIM TO LOSE BALANCE AND FALL, STRAINING HIS LEFT KNEE.

I.NAME WAS LIFTING A JACKHAMMER (WITH THE DRILL STEEL AFFIXED) OUT OF A HOLE. TO REMOVE THE JACKHAMMER WITH THE DRILL STEEL AFFIXED, HE LIFTED IT WITH ARMS EXTENDED AT EYE LEVEL, AND EXPERIENC ED PAIN IN HIS RIGHT SHOULDER AS HE DID SO. HE REPORTED THIS TO THE COMPANY NURSE ON JANUARY 4, 1990, AND WAS REFERRED FOR FURTHER MEDICAL CARE.

THE EMPLOYEE WAS DRILLING 6 FOOTDRILL STEEL INTO A QUARRY BLOCK WHEN HIS DRILL STEEL BROKE.EMPLOYEE LOST BALANCE AND FELL FORWARD OFF THE 6\HIGH BLOCK.AS HE ATTEMPTED TO BREAK HIS FALL HE SUFF SUFFERED A FRACTURE TO HIS LEFT WRIST.
