Due to severe weather conditions the scalehouse to the Quarry was closing. The employee had removed the trash and was walking back to the scalehouse when he fell in the parking area. An ambulance was called and the employee was taken to the hospital.

While lifting the hood of a water truck to do preshift inspection, employee felt a strain in his arm. Employee was able to work within the boundries of his job capacity until 10/25/04 at which time his doctor placed him out of work for further treatment of this injury.

EE DID NOT TURN THE PRESSURE OFF, THIS CAUSED PRESSURE TO BE MAINTAINE D ON THE MANUAL RELEASE. EE TOOK THE SAFETY RING OFF THE MANUEL RELEASE LENS CAUSING IT TO QUICLY MOVE FORWARD UNDER PRESSURE LACERATED EE'S HAND.

PLATE WAS FIRST LOWERED IN PLATE HOWEVER APPROX. 2 INCHES FORM FLOOR AS THE EE AND HIS FELLOW WORKER ATTEMPTED TO GUIDE T HE PLATE TO ITS RESTING POINT. THE EE CAUGHT HISRIGHT INDEX FINGER BETWEEN THE PLATE AND THE WALL. PLATE WAS LOWERED IN A HOIST.
