The Employee was trying to remove a loader tooth for a bucket, when the employee used a hammer to hit the tooth a piece of the tooth broke off and flew into the employee's leg.

The employee was helping change a screen when EE smashed a finger between the old screen being removed and the handrail. The employee received stitches in the middle finger of left hand.
Employee was helping pull a conveyor belt on when the rope broke, hitting EE on the right side of the face. Diagnostic only thru 2/15. Released for light duty on 2/18/2016.

The claimed injury was not reported to the company until 12/23/2013. The injury was a result of horseplay involving individuals' physical contact (punching, shoving, and lifting off the ground). The activity was not work related and resulted in a lower back strain (?) to an employee that previously has had back surgery.
While working on a Cat 773B Haul truck, someone bumped a 50 ton jack that was not under a load. The jack fell over striking the injured's hand and broke two fingers.

While servicing a water pump on a highway drill, a faulty hydraulic motor engaged while the driller was inspecting the belts and pulleys. His three fingers (index, middle and ring) were trapped between the belt and pulley severing the tips to the first knuckle.

While cleaning out the Hazmag Crusher, the injured threw a rock out and caught his finger between the crusher side and the rock, mashing his finger requiring stitches to close.

While dismounting from a Bobcat during rain and muddy conditions, the EE's foot slipped causing him to fall trapping his leg and injuring his right knee. He has continued to work on light duty "Restricted Work Activity" and will be operated on 01/27/10.

PLANT OPERATOR WAS CHIPPING ON A HAMMER ON THE SPOKANE CRUSHER. WHEN EE APPROACHED AREA IN HIS PICK UP TRUCK TO DELIVER BOLTS. SOMETHING FLEW INSIDE CAB STRIKING HIS IN EYE. THOUGHT HE HAD REM OVED OBJECT, HOWEVER, PAIN INCREASED AND LATER SOUGHT MEDICAL TREATMENT FOR EYE INJURY.
EE TIGHTENING BOLT ON ROCK CRUSHER. THE WRENCH SLIPPED OFF AND HE FELL BACKWARDS HITTING HANDRAIL. HE CONTINUED FALLING BACKWARDS AND CAUGHT FULL WEIGHT OF BODY ON LEFT WRIST EXTENDED BEHIND H IMSELF TO BREAK FALL ONTO WALKWAY.

WHILE STRIKING A TRACK RAIL WITH A HAMMER, A PIECE OF THE RAIL CHIPPED OFF AND STRUCK LEFT HAND CUTTING NEAR THUMB.
EE STUCK FINGER INTO A.C. FAN CAUSING CUTS REQUIRING STITCHES TO MIDDLE FINGER OF RIGHT HAND. TRUCK HAD JUST FINISHED INSPECTION BY MSHA NO LESSTHAN 30 MIN. PRIOR TO ACCIDENT.
