Propane leak. Gas buildup under crusher feeder. EE changed propane tanks w/o turning off the burner valve which caused a build up of gas when he lit the burner it caused gas build up to flash which caused 2nd degree burns to right arm & face.

HIT HEAD ON CANOPY WHEN GETTING INTO CAT. HE WASPUSHING ROCK IN PIT. HAD STROKE WENT TO HOSPITAL- EE STATES THAT HITTING HIS HEAD ON CANOPY CAUSED STROKE.
WE WERE MOVING EQUIPMENT. I WAS HOLDING A BOARD AGAINST THE SHOP VAN SO WE COULD PUT THE SS AGAINST IT TO HOLD PRESSURE. SO WE COULD GET THE FIFTH WHEEL UNDER UT. THE BOARD BROKE AND PINCHE D MY FINGER AGAINST THE SHOP VAN.
I AND ANOTHER GUY WERE PRYING WITH A BAR ON A ROCK THAT WAS WEDGED INSIDE THE JAW CRUSHER THE BAR SLIPPED AND THE END I HAD MY HAND ON FORCED MY HAND INTO THE SIDE OF THE FEED APRON AND IT SMA SHED THELITTLE FINGER AND SPLIT IT OPEN FOR THE MIDDLE OF THE NAIL TO 1/4" DOWN ON OTHER SIDE
