Two employees were trying to open a hinged door on the mill for the purpose of flipping the hammer. During the procedure of opening the door a 5/16" chain being used became unlatched at the center of gravity break over. Neither of the two employees realized the chain had come unlatched

REMOVING PIPE ON HIGHWALL EE WAS WORKING TOO CLOSE TO THE MACHINE TO BE DIRECTING THE OPERATOR. THE EE WAS ON THE INSIDE OF THE BERM WITH THE MACHINE WHICH COULD BE A RESTRICTED WORK AREA.
WAS MOVING ELECTRIC STARTERIN SITCH HOUSE. THE STARTER GOT UNBALANCED. HE PUSHED HIS LEFT LEG OUT HITTING HINGES ON OLD STARTER CUTTING ONE INCH ON HIS LEFT CHIN BONE.
EE WAS TIGHTENING BELTS ON ALTERNATOR OF HIS HAUL TRUCK USING A 12" PRY BAR. THE BAR SLIPPED & CAME DOWN HITTING EE ABOVE TH RIGHT EYE CAUSING A SMALL CUT THAT REQUIRED 5 STITCHES

EMPLOYEE LOST CONTROL OF WATER HOSE. CHANGING WATE RPRESSURE DUE TO USE OF SEVERAL HOSES MAY HAVE SURGED EMPLOYEES HOSE, CAUSING IT TO STRIKE HIM IN THE FACE. INJURY WAS IN THE NOSE AREA. NO SE BLEED BEGAN AFTER HOURS AT WHICH TIME A POSSIBLE FRACTURE TO SINUS BONE WAS DICOVERED.
