Loader operator laid out a small sample pile of rock for the QA inspector and then forgot it was there and ran over it.

Technician was repairing machine. Technician stepped backwards and heel got caught on frozen ground and fell back injuring left shoulder.

Employee was removing forklift attachment from a skid-steer loader and the attachment shifted when it came loose and pinched two of his fingers on the left hand causing cuts the length of both the ring and middle finger.

Three men were using pry bars to roll over track chain to be installed on EX-650D excavator. When they rolled it over, the pry bar came out of the chain and hit the ground, bouncing back up & striking employee on the front side of his hard hat. He received 1.5" cut on his forehead. This produced minor bleeding from the cut.

Employee was getting pipe off of overhead storage rack and the 3 pipes fell the first hit his hard-hat and knocked it off and the 2nd struck him in the head.

Employee was begining maintenence on a crusher. He was lifting the top and it slipped from his hands and fell on his leg and foot.

Bulldozer operator came into office and requested an ambulance be called. He then laid down on the floor. He remained concious until the ambulance arrived and transported him to the hospital.

Employee was changing a hydraulic hose on a Caterpillar excavator and a pry bar slipped and it struck him in the face.

Employee stepped over boulders used as temporary restraining near a newly constructed wall. When he stepped over he slipped on mud and slid several feet and then fell over the approx. 18 foot high concrete wall. Landing on his feet.

Employee was preparing to change drive sheave on a new Tabor 3 deck screen. Machine was locked out. He removed the guard and started to reach in and the ecentric cam rolled causing the pulley to turn 1/2 of a turn and caught his hand between the sheave and the belts.

Employee was performing maintainence on a bullzozer and caught his left ring finger between a cylinder and a pin, pinching his finger that required stitches.

Employee was working to repair a conveyor belt from a bucket truck. After he got down his back was hurting. He finished his shift and went home for the weekend. During work on Monday it began to hurt more and he went home that afternoon.

Employee was cutting a piece of rubber belting and the knife slipped and cut his leg.

EE CALLED AT BEGINING OF SHIFT AND SAID HE COULD NOT COME TO WORK. HE ORIGINALLY TURNED THE CALIM INTO HIS PERSONAL INSURANCE. ABOUT 2 MONTHS LATER THE DR SAID THIS WAS A REINJURY OF AN INJURY THAT OCCURRED 10-29-99.

CONTRACTOR WAS CHANGING A TIRE ON A HAUL TRUCK. WHEN TIRE/RIM CAME OFF OF HUB, TIRE TIPPED OVER AND FELL ON LEG OF CONTRACTOR.

SERVICING TIRE AT QUALITY LIME ON EQUIPMENT TIRE FELL AND PINNED AGAINST TRUCK, CUT LEG ON STEPS ON TRUCK.

EE WAS SPLICING CONVEYOR BELT, HE WAS USING A PAIR OF LOCKING PLIERS THAT SPRUNG LOOSE & HIT HIM IN THE LIP.

EMPLOYEE SLIPPED ON ICE AND FELL AND BROKE HIS FOOT.

EMPLOYEE WAS HAMMERING RIPPER TOOTH ON BULLDOZER AND A SMALL PIECE OF METAL WENT INTO HIS THUMB.

EMPLOYEE WAS WORKING WITH ANOTHER EMPLOYEE REPAIRING EQUIPMENT. OTHER EMPLOYEE STRUCK EFFECTED EMPLOYEE IN THE LEG WITH A SLEDGEHAMMER.

EMPL WAS CARRYING SMALL ELECTRIC MOTOR UP WALKWAY & SLIPPED ON PATCH OF ICE STRAINING BACK.

EMPLOYEE WAS WORKING ON 2 LADDER AND SLIPPED AND FELL ONTO FROZEN ROCK PILE.

EMPLOYEE WAS ENERGIZING AN ELECTRICAL HEATER WHEN THE SWITCH MALFUNCTIONED AND CAUSED A SMALL FIRE.

EMPL WAS UNWINDING EXTENSION CORD AND THE END FLIPPED UP AND HIT HIM IN THE TOOTH

EMPLOYEE WAS WORKING ON CONVEYOR AND FELL 5 FT INTO THE BACK OF A PICKUP TRUCK AND LANDED ON A BOARD.

EMPLOYEE WAS PU7TING BIT ONTO KELLY BAR OF DRILL WHEN THE WRENCH HE WAS USING SMASHED HIS FOOT.

EMPLOYEE REACHED TOWARD ELECTRICAL BOX TO UNPLUG 440 ELECTRIC HEATER. RIGHT HAND WAS WITHIN 2 FEET OF BOS WHEN IT EXPLODED CAUSING BURNS TO HIS RIGHT HAND.

EMPLOYEE WAS IN THE PROCESS OF CHANGING THE SEALON A WATER PUMP WHEN HE WAS REMOVING A BELT WHICH ROLLED OFF THE PULLEY AND CAUGHT HIS FINGER IN THE PULLEY.

EMP TRIED TO JUMP ACROSS A DITCH WHILE CARRYING LARGE WATER HOSE THE NEXT DAY EMP FELL INTO A HOLE AGGRAVATING THE INJURY

OPERATOR WAS CLIMBING INTO CAB OF ELEVATING SCRAPER HE HAD A HOLD OF THE DOOR FRAME WHEN THE WIND BLEW TH*E DOOR SHUT ON HIS LEFT THUMB LACERATING IT SEVERELY
