EE WAS CHANGING OUT A BROKEN BOLT ON PRESS ROLLER TRACKING ARM. THE OTHER BOLT HOLDING ARM BROKE & ROLLER FELL AGAINST HIS ARM CAUSING A BRUISE.
EMPLOYEE IS A SURFACE MINE FOREMAN. HE WAS RE-SETTING A LIGHT PLANT TO A NEW SURFACE LOCATION. HE RAISED THE BOOM ON PLANT AND REACHED DOWN TO SET HOLDING PIN. THE BOOM SLIPPED OUT OF HIS HAND PINCHING HIS LEFT INDEX FINGER BETWEEN BOOM AND PINCH POINT.
